B型急性大動脈解離の発症２年３ヶ月後に、血管径拡大に対し上行全弓部置換術を施行した。この時の切除標本から大動脈炎の合併と診断された。手術から１年後、遠位弓部血管径の拡大あり。追加で下行大動脈に胸部ステントグラフト内挿術（Thoracic Endovascular Aortic Repair:TEVAR）を行った。TEVAR後半年、エンドリークは無く瘤径は縮小傾向であったが、偽腔内に気泡像が出現した。微熱と炎症値の軽度上昇を認め、グラフト感染の診断で入院の上で抗生剤を開始した。抗生剤開始直後、発熱は軽減し炎症値も低下したが、微熱の残存と炎症値の陽性は改善せず。大動脈炎に対する治療として、プレドニゾロンを開始した。プレドニゾロン使用３ヶ月後、偽腔内の気泡像は消失し、炎症値も陰性化したため、抗生剤とプレドニゾロンの使用を中止した。TEVARから４年半を経た現在も感染の再燃は認めていない。TEVAR後に偽腔内に気泡像の発生が認められ炎症所見が遷延した慢性解離性瘤に対して保存的な治療のみで寛解を得たため文献的な考察を加えて報告する。","subitem_description_type":"Other"}]},"item_3_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{"nameIdentifier":"13891","nameIdentifierScheme":"WEKO"}],"names":[{"name":"Nakamura,
